FacilityDirectory.org is an independent, informational directory of Medicare- and Medicaid-certified nursing homes, home health agencies, and hospice providers in the United States. Our goal is simple: make the public data that already exists about these providers easier to find, read, and compare.
Who we are
We are a small editorial and engineering team that builds data tools for families, caregivers, researchers, journalists, and advocates. We are not a healthcare provider, a placement agency, a referral service, or a government body. We do not sell care, and we do not accept payment to change how a provider appears in the directory.
What we publish

How to use the site
Use the directory to narrow a list of providers worth calling or visiting, and use the source data to prepare questions. Data can be incomplete or lag behind real-world conditions. Always confirm details directly with the provider and with the state agency that licenses it.

If you find an error, please tell us. We correct factual mistakes in our own content and re-pull data when CMS publishes updates. See the contact page.
